What the difference other than 3 vs 6 month suspension ?

They mean the same thing and don't relate to the length of the suspension. A horse is suspended for 90 days the first time it bleeds out through one or both nostrils, 180 days the second time, and permanently the third time. Epistaxis is a fancy word for nosebleed.

is there any possibility that a horse is more likely to bleed at sea level than at Altitude?
And is bleeding more frequent at Sea Level vs. Altitude?
And is bleeding more frequent at Sea Level vs. Altitude?

In my time working with horses I have found that horses bleed more often when 1. You work hard on a heavy or dead track 2.You are working with another horse so they become more competitive.

I haven't done the numbers, but there is a strong school of thought that that is exactly the case. It may help to explain why a notorious bleeder like English Garden keeps turning up in Joburg.
